OHSAA Preparticipation Physical Evaluation Form
NCAA Eligibility Center
All students who plan to enroll in a Division I or Division II college and want to participate in athletics or receive an athletic scholarship must register with the NCAA Eligibility Center.
You may register on-line at www.eligibilitycenter.org, click on spring 2010, then click on new account, and follow the prompts. You will need a credit card and its expiration date, because there is a $60 fee to register.
You will also need your social security number, the high school code (360368), your high school graduation date (5/31/11).
You will need to notify the guidance office after you have registered and fill out a transcript request form in order for your official transcript to be sent to the Eligibility Center.
